Thank you for sharing your time and love with your foster dog! Any questions regarding the foster program, please contact the Planned Pethood Dog Foster Coordinator

 By signing this application, you are agreeing that ownership of the dog you foster remains solely with Planned Pethood, Inc. until such time as adopted. All of the information that you have provided must be filled out truthfully to the best of your knowledge. You also are agreeing that you will treat the dog as a member of your family, not abusing or letting anyone else abuse the dog.

You will provide food, water, shelter and attention for the dog. You agree to make a commitment to care for a dog until a permanent home can be located. The medical care of the dog is the sole responsibility of Planned Pethood, Inc. You understand the Dog Foster Coordinator or the Dog Intake Coordinator must be contacted for permission to obtain any medical care needed. You also agree to utilize the medical facilities approved by Planned Pethood, Inc.

Before fostering begins you will meet with a representative from Planned Pethood and sign a fostering contract.

Set up times for our adoption events begin at 10:30am with clean up/tear down at 2:45pm. Events usually start at 11am and run until 3pm. Knowing this, will you be able to attend most of our adoption events?
